Collene Janice Samuels, 34, of Green Cove Springs, FL, passed away Friday, April 1, 2011. She was born on September 25, 1976 in Hammond, IN. She was a beloved wife and mother who will be dearly missed by her loving family and friends. She was preceded in death by her parents and is survived by her husband, Shandale Samuels; children, Daniel Dickens, Molly Hastie and Cruz Linda Samuels; sister, Charlene; mother-in-law, Linda Samuels; sister-in-law, Lynscena Samuels and brother-in-law, Gregory Samuels.
