Chad Amic Crutchfield passed away Sunday morning, March 25th, 2012. In his last days, he was surrounded by many loving friends and family. His wife and great friend, Hooch, held his hands until Chad's 17 month battle with leukemia came to an end. He is finally at peace.
Chad was an employee of Miller Electric as a union electrician for the last 22 years. He was the most amazing husband, father, sibling, son and friend. His valiant spirit and positive attitude through this journey inspired all that has had the pleasure of having him in their lives.
He is survived by his loving wife, Andrea Crutchfield. His children: beautiful daughters, Madison and Kalei Crutchfield, handsome son, Nathan Crutchfield and stepson, Brandon Blansit. His parents: Kelly and Jackie Crutchfield and June Butler. His siblings: Dana Parr (Stacey), Kelly Crutchfield (Lisa), Joel Crutchfield, Chris Crutchfield (Laurie), Todd Crutchfield (Crystal), Joey Butler and Janie Davis. His mother and father in law, Charles and Donna Burtnett, sister's-in-law, Liz Fiala (Jody), Denise Gosney (Tom), brother in law, Bret Burtnett and many nieces, nephews and the most wonderful friends ever.

Memorial services will be held on Friday, March 30, 2012 at 11:00 at First Baptist Church of Middleburg, 2645 Blanding Blvd., Middleburg, FL, 32068 with Pastor Ralph officiating and Robert Dallatore eulogizing. Following the service, family and friends are welcome to celebrate Chad's life with us at Whitey's Fish Camp, 2032 CR 220, Orange Park, FL 32003.
