Terry Lynn Chojnacki

Terry Lynn Chojnacki, 62, lifelong resident of Orange Park, FL passed away in her home on October 11, 2013. She was preceded in death by her father, Hubert “Cubie” Chojnacki. She is survived by her mother, Gwen Chojnacki; brother, Kim Thomas Chojnacki; sister, Dawn Miller; cousin, friend and caretaker, Paula Wright; as well as numerous other cousins and friends. The family has asked donations be made to Northeast Florida Community Hospice. Funeral Services will be held at 11 a.m. on Monday, October 14, 2013 in the Broadus-Raines Chapel with Fr.
