Derrick "Chad" Chadwick

Derrick “Chad ”Chadwick, 60, of Jacksonville, FL, passed away November 20, 2014. He was born on January 8, 1954 in Afragola, Italy to parents Charles and JoJ Malden Chadwick. Chad had lived in the local area for 54 years and had worked in the Dietary Department for Signature Health Care. He was a member of Riverstone Community Church and the American Legion. Chad is survived by his mother JoJ Porter; brother, Lance Chadwick and sister, Lyndalea “Linda” Crane. A memorial service will be held at Judy’s Pub, 5800 San Juan Ave., Jacksonville, FL 32210 at 2:00 pm on Sunday, November 30, 2014. In lieu of flowers, memorial contributions in his name may be made to Wolfson Children’s Hospital, 800 Prudential Dr., Jacksonville, FL 32207.
