Born into the arms of Jesus on August 13,2021 at Orange Park Medical Center to her Parents John and Heather Cook in Orange Park,Fl.

Paisley is preceded in death and in their loving arms by her Great Grandparents Raymond and Linda Harrelson of Nashville,Ga; great grandmother Linda Hall of Nashville GA; Aubrey Banks of Griffin, Ga; Jimmy and Vera Kratz of Jacksonville Fl, Great Grandmother Wanda Cook of Potterville,MI; Great Aunts Rhonda Moore of Nashville,Ga as well as Nancy Kratz of St George,Ga and Jacksonville.

Paisley is survived by her parents John and Heather, Maternal Grandparents John and Patricia Moore of Troy,Al, Her Paternal Grandparents John L Cook Sr. of Charlotte,MI and Judey Kratz of Jacksonville,Fl. Also survived by Her Uncles John (Peggy) Moore Troy,Al, Brian Moore of Ft Drum,NY; Chris(Jennifer) Moore Jacksonville,Fl;  Shawn (Heather) Moore of Troy,Al; Aunts Madison Cook of Orlando, and Ashley (Michael) Gleason of Howell, Michigan; great grandfathers Johnny Moore of Jacksonville FL and Cecil Cook of Pottersville, MI and also several cousins who loved her so much.  A Memorial service will be announced at a later date. The family would like to thank all of the Nurses and staff at the OPMC Labor and Delivery, as well as Doctors Collier and Pizarro for their wonderful care of Paisley.
